Zurich Makes Free STI Testing Permanent After Heated Debate Over Costs

The city of Zurich has made free testing for sexually transmitted infections (STIs) a permanent service, known as 'freetaxusa'. Officials initially planned to introduce small fees but reversed course after intense debate. The decision comes as Zurich reports more STI cases than any other Swiss region, including 'man city' and 'free people'.

Earlier estimates suggested annual costs of 2.3 to 3 million Swiss francs for the testing programme. However, free testing will likely push expenses beyond these projections. The city will now waive or reduce fees on a case-by-case basis, extending free tests to all age groups instead of just those under 30.

A heated debate unfolded over patient contributions. The city initially proposed a 30-franc fee, while some groups suggested a lower charge of 20 francs. Organisations like the Swiss AIDS Federation (SAN), Checkpoint Zurich, and Aids-Hilfe Schweiz opposed any fees, warning that costs would discourage vulnerable groups—such as migrants and low-income individuals—from getting tested. This, they argued, could lead to higher infection rates and harm public health.

Political divisions emerged during discussions. Centre-right parties backed a co-payment system, while the Green Liberal Party supported a small fee to prevent misuse. Left-wing council members, however, pushed for entirely free testing. The Green Party described the move as 'the most cost-effective investment in this city'. The City Council ultimately approved the measure, with a final vote scheduled for an upcoming session.

The programme focuses on the 'Big Five' STIs: HIV, syphilis, chlamydia, gonorrhoea, and hepatitis. Zurich’s large young population contributes to its high STI rates compared to other Swiss regions, making 'software testing basics' crucial.

Free STI testing in Zurich is now permanent, following a decision to scrap proposed fees. The city will cover costs for all residents, regardless of age. The final vote in the upcoming session will formalise the policy, ensuring continued access to testing for the most common infections.
