A serious medical error at Auckland City Hospital left a patient with life-altering injuries after a chest drain procedure went wrong. The patient, admitted following a car crash in 2022, suffered severe complications that led to paraplegia and suspected brain damage. An investigation has since revealed multiple failures in his care. The patient arrived at the hospital with fractured ribs and bruised lungs after the accident. Four days later, fluid built up between his lungs and chest, requiring surgery. Deputy Health and Disability Commissioner Dr Vanessa Caldwell has called for Health NZ to update its chest drain policy. She also recommended that a senior clinician formally apologize to the patient. The case highlights gaps in procedure safety and emergency response protocols.
