Additional Contributions 2026: How Much Your Health Insurer Costs in Comparison - German health insurance costs rise sharply in 2026 for millions
Many Germans will pay more for health insurance in 2026. Half of the country’s insurers have raised their additional contribution rates since the start of the year. The average surcharge for members climbed from 2.94% to 3.13%, adding extra costs for policyholders across the country.
The cheapest provider this year is BKK firmus, with a 2.18% surcharge. At the other end, BKK 24 charges the highest rate at 4.39%. Other costly options include IKK Brandenburg und Berlin (4.35%), IKK – Die Innovationskasse (4.3%), and Energie BKK (3.98%). Some, like BKK exklusiv, increased their rates sharply—by up to 1.1 percentage points.
Policyholders facing higher fees can still switch providers. The law allows them to cancel their contract by the end of the month when the new rate first applies, with a two-month notice period. Choosing a cheaper insurer could save employees between €240 and €770 per year, depending on their salary and the difference in rates. Beyond cost, insurers offer different perks that may influence decisions. Some provide 24/7 phone support, app-based services, or local branches for in-person help. Others cover extras like dental cleaning subsidies or osteopathy treatments, which can add up to real savings over time.
The changes mean higher costs for millions, but switching remains an option for those looking to cut expenses. With surcharges now ranging from 2.18% to over 4.3%, comparing providers could lead to significant annual savings. Policyholders must weigh both price and benefits before making a decision.
